Purpose of the Playground Consent Form
This form is used to obtain permission for a child to participate in activities at the school playground or during school-organized visits to local playgrounds. It ensures that parents or guardians are aware of the nature of these activities, the safety measures in place, and any associated risks. Parents or guardians must review and sign this form to allow their child to engage in playground activities.
Parent/Guardian Consent:
By signing this form, I, the undersigned parent or guardian of the named child, acknowledge and agree to the following:
- Permission for Playground Activities
I grant permission for my child to participate in playground activities during school hours or during school-organized outings to local playgrounds. I understand that my child will be supervised by school staff during these activities.
- Assumption of Risk
I understand that playground activities may involve certain risks, including minor falls, scrapes, or injuries that can occur during physical play. I acknowledge that the school will take reasonable measures to ensure my child’s safety but cannot eliminate all risks associated with outdoor play.
- Responsibility for Safety
I understand that it is my responsibility to inform the school of any medical conditions, physical limitations, or special needs my child may have that could impact their participation in playground activities.
- Medical Consent
In the event of an injury or emergency, I authorize school staff to provide basic first aid to my child and to seek further medical care if needed. I understand that I am responsible for any medical expenses that may result from such care.
- Release of Liability
I release the school, its employees, and representatives from any liability or claims related to my child’s participation in playground activities, provided they have acted in good faith and with reasonable care.
- Revocation of Consent
I understand that I may revoke this consent at any time by providing written notice to the school. However, I acknowledge that such revocation may impact my child’s participation in playground activities.
- Automatic Denial in Absence of Form
I understand that if this form is not completed and signed, my child will not be allowed to participate in playground activities.
