ELEMENTARY SCHOOL WAIVER AND RELEASE OF LIABILITY
I, the undersigned, as the parent/legal guardian of the participant (“Child”), acknowledge and understand the nature of the Elementary School Fun Run (“Event”). I hereby grant permission for my Child to participate in the Event.
- Assumption of Risk: I recognize and acknowledge that there are inherent risks associated with participating in the Event. I have fully informed myself of the details of the Event and the potential risks involved in my Child’s participation, and I willingly and voluntarily assume all such risks on behalf of my Child.
- Release of Liability: I hereby release, waive, discharge, and covenant not to sue the Elementary School, its trustees, officers, employees, sponsors, organizers, volunteers, or other representatives, from any and all liabilities, claims, demands, actions, causes of action, costs, and expenses directly or indirectly arising out of or related to any loss, damage, injury, or death, that may be sustained by my Child or to any property belonging to my Child, while participating in the Event, or while on the premises where the Event is being conducted.
- Medical Treatment: In the event of an injury, I authorize the Elementary School to seek, administer, or have administered whatever first aid or medical care deemed necessary for my Child’s welfare, and I shall be responsible for any and all costs of such medical attention and treatment.
- Photography and Videography: I understand that photographs, videos, and other recordings may be taken during the Event. I hereby grant permission for my Child to be photographed, videotaped, or otherwise recorded for the purposes of promoting the Event or the Elementary School. I waive any right to compensation or approval of the finished photographs or recordings.
- Acknowledgment: I have read this waiver and release of liability, fully understand its terms, and understand that I am giving up substantial rights by signing it. I acknowledge that I am signing this waiver freely and voluntarily and intend my signature to be a complete and unconditional release of all liability, including that due to inherent risks or the negligence of the released parties, to the greatest extent allowed by law.
