Purpose of the COVID-19 Parental Acknowledgement Form
This form is used to confirm that parents or guardians are aware of the school’s COVID-19 policies and guidelines and the associated responsibilities for their child’s safety and health. It acknowledges the risks of COVID-19 exposure and the parent’s role in monitoring their child’s health and following the school's safety procedures.

By signing this form, I, the undersigned parent or guardian of the named child, acknowledge and agree to the following:
- Awareness of COVID-19 Risks
I understand that COVID-19 is a contagious virus that can spread through person-to-person contact. I acknowledge that while the school is implementing health and safety measures to reduce the risk of transmission, it is not possible to completely eliminate the risk of exposure to COVID-19 within the school environment.
- Health Screening Responsibilities
I agree to monitor my child for symptoms of COVID-19 daily, including fever, cough, shortness of breath, or any other symptoms outlined by public health guidelines. I will keep my child at home if they display any symptoms of illness or if they have been in close contact with a confirmed case of COVID-19.
- Compliance with School Policies
I agree to follow the school’s COVID-19 policies and procedures, including any requirements for masks, physical distancing, hand hygiene, and any other measures implemented by the school to ensure a safe environment. I will ensure that my child understands and follows these protocols.
- Notification of Exposure
I agree to notify the school immediately if my child tests positive for COVID-19 or has been in close contact with a person who has tested positive. I understand that this is necessary to protect the health and safety of other students, staff, and families.
- Remote Learning and School Closure
I understand that in the event of a COVID-19 outbreak or public health directive, the school may transition to remote learning, and my child may need to participate in online instruction as required.
- Release of Liability
I release the school, its employees, and representatives from any liability or claims related to my child’s potential exposure to COVID-19, recognizing that the school is taking reasonable steps to mitigate the risks.
- Automatic Assumption in Absence of Form
I understand that if this form is not completed and signed, the school will assume that I do not agree to the terms outlined, and this may impact my child’s participation in on-campus activities.
